CGHS card from Central Government to benefit its employee under health schemes. This plastic card provides Central Government Health Scheme which is valid under the reason of CGHS boundaries. An employee moved to new location from his current working, need to make sure the CGHS Area limit.
If the new location isn’t falling in boundaries of current CGHS department, then ID may transfer to their department. This is mandatory to transfer the card to a new location to avail the benefits. Failing to do will lead to cancellation of CGHS Card. No benefits may taken by an employee or their family members.
Transfer of CGHS is mandatory task for Government employee. To make sure their names fall under health wellness center, and the wellness centers provide a wide range of health benefits to employees and their family members. The card issued to any employee is valid only under the region of their particular department.
CGHS Card Transfer
The process to transfer the card is quite easy and simple with simple steps as mentioned below, if arranged the documents as mentioned
- Employees need to have the transfer Certificate from AD
- Fill the application form with details along with family members names
- Submit CGHS plastic card along with application form and TC to current department
- Receive Employee Surrender Certificate
- Submit Valid ID Proofs
- Application Form for Transfer of CGHS Card may download from Central Government Health Official website and Transfer certificate of Employee may issued by the Head of department.

I am a pensioner holding CGHS card for me and for my wife for CGSH dispensory No 63 Sahique Nagar New Delhi 110048 for my address E250 Greater Kailash 1 New Delhi.
Both of us are facing difficulty for following reasons:-
1 Road crossing from GK1 to sadiqu nagar has been clossed by delhi police.
2 Dispensary lift is out of order for the past three years and no action by any authority.
Kindly shift my cards to dispensary no 39 Andrus Ganj and oblize.

My husband (Pensioner) and myself (spouse) CGHS cards have been issued by Begumpeth Hyderabad CGHS office.
We had shifted to Jabalpur last year but due to ill health he expired on 05.12.2022 in Jabalpur.
